Iniparib + chemotherapy refers to combinations in which **iniparib**, a small molecule investigational anti-cancer agent, is administered alongside DNA-damaging chemotherapeutics, notably **irinotecan**, **carboplatin**, and **gemcitabine**. Iniparib was originally characterized as a poly(ADP-ribose) polymerase (PARP) inhibitor, but further studies did not support this mechanism. Instead, iniparib is now generally believed to act by altering reactive oxygen species (ROS) metabolism and inducing DNA damage in tumor cells, causing cell cycle arrest at G2/M and potentiating the effects of DNA-damaging agents. Combination regimens have been studied primarily in advanced **triple-negative breast cancer** (TNBC) with brain metastases and recurrent **ovarian cancer**. Clinical trials revealed modest activity in TNBC brain metastases and ovarian cancer, with the strongest responses noted in patients with germline BRCA mutations. The combination is typically administered intravenously, commonly with irinotecan or gemcitabine and carboplatin[1][2][3][4]. Iniparib is not currently approved for any indication.
