Overview

**iNO45** is a specific pulsed inhaled nitric oxide (iNO) dose of 45 mcg/kg ideal body weight per hour delivered via the proprietary INOpulse device for patients with fibrotic interstitial lung disease (fILD), particularly those at risk of pulmonary hypertension (PH) on supplemental oxygen. Nitric oxide acts as a selective pulmonary vasodilator, improving ventilation-perfusion matching with minimal systemic effects due to its short half-life. Developed by Bellerophon Therapeutics, it showed positive Phase 2 results with significant improvements in moderate to vigorous physical activity (MVPA) and patient-reported outcomes like dyspnea and quality of life, but the Phase 3 REBUILD trial failed to meet its primary MVPA endpoint, performing worse than placebo, leading to no further development path.[1][2][3]
