Drug intelligence / Profile preview
**INOpulse** is a portable drug-device combination product developed by Bellerophon Therapeutics (later associated with Mallinckrodt) that delivers pulsed inhaled nitric oxide (iNO) via a proprietary triple-lumen nasal cannula synchronized to the patient's breathing. As a potent pulmonary vasodilator, iNO selectively targets well-ventilated lung regions to improve ventilation-perfusion matching, enhance oxygenation, reduce pulmonary vascular resistance, and lower pulmonary arterial pressure with minimal systemic effects due to rapid inactivation in blood. It has shown promise in Phase 2 trials for maintaining moderate-to-vigorous physical activity, stabilizing symptoms like dyspnea, and improving hemodynamics in patients with fibrotic interstitial lung disease (fILD), pulmonary hypertension associated with interstitial lung disease (PH-ILD), pulmonary hypertension associated with pulmonary fibrosis (PH-PF), pulmonary arterial hypertension (PAH), and chronic obstructive pulmonary disease-associated pulmonary hypertension (COPD-PH), particularly those requiring supplemental oxygen. A Phase 3 trial in fILD was underway as of the latest reports, with FDA orphan drug designation for PAH and idiopathic pulmonary fibrosis.[1][2][3][5][7]
