Inosine pranobex (also known as inosine acedoben dimepranol or methisoprinol) is a synthetic compound that combines inosine with dimepranol acedoben (a salt of p-acetamidobenzoic acid and N,N-dimethylamino-2-propanol) in a 1:3 molar ratio. It functions as both an immunomodulator and an antiviral agent. Its primary mechanism involves stimulating the cell-mediated immune response, specifically by enhancing T-lymphocyte differentiation and proliferation, increasing natural killer (NK) cell activity, and promoting the production of pro-inflammatory cytokines such as interleukin-1 (IL-1), interleukin-2 (IL-2), and interferon-gamma (IFN-γ). Additionally, it exerts antiviral effects by inhibiting viral RNA synthesis and translation. Originally developed by Newport Pharmaceuticals, it has been extensively studied for conditions like AIDS-related complex (ARC), though it is currently most commonly used for subacute sclerosing panencephalitis (SSPE), herpes simplex, and various viral respiratory infections.
