Inotuzumab ozogamicin is an antibody-drug conjugate (ADC) used to treat B-cell precursor acute lymphoblastic leukemia (ALL), particularly in cases that are relapsed or refractory. It consists of a humanized monoclonal IgG4 antibody targeting the CD22 antigen on B cells, covalently linked to the cytotoxic agent N-acetyl-gamma-calicheamicin via an acid-cleavable linker. Upon binding to CD22-expressing tumor cells, the drug-antibody complex is internalized and trafficked into lysosomes where the linker is hydrolyzed, releasing calicheamicin inside the cell. Calicheamicin binds DNA and induces double-strand breaks, leading to cell cycle arrest and apoptosis. Inotuzumab ozogamicin was developed by Wyeth (now part of Pfizer) and is marketed under the brand name Besponsa[3][5][6][7][8].
