Overview

INP104 is a non‑oral, intranasal drug–device combination product consisting of dihydroergotamine mesylate (DHE) delivered to the upper nasal space using Impel’s Precision Olfactory Delivery (POD) technology for the acute treatment of migraine with or without aura in adults.[1][2][3] As an ergot derivative, DHE is a non‑selective serotonin receptor agonist that causes vasoconstriction of intracranial blood vessels and inhibition of neurogenic inflammation, providing rapid relief of migraine pain and associated symptoms; upper‑nasal targeting enables fast, consistent systemic absorption with bioavailability and early exposure comparable to intravenous DHE but in a self‑administered nasal spray format.[2][3][4] The product was developed by Impel NeuroPharma (now Impel Pharmaceuticals) and approved by the US FDA under the brand name Trudhesa as the first therapy to use POD technology, with a lower DHE dose than older nasal formulations and a favorable long‑term safety and tolerability profile demonstrated in the Phase 3 STOP 301 study.[1][2][3][10]
