InsuLAR (PB1023) is an ultra-long-acting basal insulin analogue developed by PhaseBio Pharmaceuticals for the management of type 2 diabetes mellitus. The drug is a recombinant fusion protein comprising human insulin and PhaseBio's proprietary Elastin-Like Polypeptide (ELP) technology. Upon subcutaneous administration, the ELP-insulin fusion protein undergoes a temperature-dependent phase transition at body temperature, forming a reversible coacervate depot at the injection site. This depot slowly releases the insulin-ELP molecules into the systemic circulation, where they act as agonists at the insulin receptor to promote glucose uptake and suppress hepatic glucose production. This unique delivery mechanism is designed to support a once-weekly dosing schedule, offering a significant reduction in injection frequency compared to standard-of-care daily basal insulins. While Phase 2a clinical trials demonstrated proof-of-concept, the development of InsuLAR was deprioritized by PhaseBio in favor of other pipeline candidates prior to the company's liquidation.
