TGC-Prince of Songkla University-hyperglycemia refers to a specific therapeutic protocol for intraoperative tight glycaemic control (TGC) developed and evaluated by researchers at the Prince of Songkla University. The intervention utilizes a hyperinsulinaemic normoglycaemic clamp technique, which involves the continuous intravenous infusion of insulin (specifically Humulin R) at a fixed rate, supplemented by a separate infusion of a modified glucose-insulin-potassium (GIK) solution. This GIK mixture contains 25% glucose, potassium chloride, and magnesium sulfate, and is titrated to maintain blood glucose levels within a target range of 80-150 mg/dL. The protocol is primarily indicated for patients undergoing cardiac surgery to mitigate the risks of postoperative infection, morbidity, and mortality associated with intraoperative hyperglycemia.
