Overview

Insulin 338 is a long-acting basal insulin analogue developed for oral administration in the treatment of type 1 and type 2 diabetes mellitus. It was formulated as a tablet using the absorption enhancer sodium caprate to improve gastrointestinal uptake. The drug features an albumin-binding motif and reduced endocytosis to extend its half-life (approximately three days in humans). Its mechanism of action is as an agonist at the [insulin receptor](https://en.wikipedia.org/wiki/Insulin_receptor) (INSR), stimulating downstream pathways such as protein tyrosine kinase activity. Clinical trials demonstrated that oral insulin 338 provided glycemic control comparable to subcutaneous insulin glargine in patients with type 2 diabetes. However, development was discontinued due to high required doses and commercial nonviability[1][2][5][6][7].
