Overview

A combination of **insulin detemir** (a long-acting human insulin analog) and **exenatide** (a glucagon-like peptide-1 receptor agonist), both administered by subcutaneous injection, evaluated as co-therapy for type 2 diabetes mellitus. *Insulin detemir* lowers blood glucose by directly binding to and activating the insulin receptor, promoting glucose uptake and inhibiting hepatic glucose production. *Exenatide* acts as a GLP-1 receptor agonist, stimulating glucose-dependent insulin secretion, suppressing inappropriate glucagon secretion, slowing gastric emptying, and reducing appetite. Though widely used together in clinical practice, there is no fixed-dose co-formulated product currently available; they are administered as separate injections. Used primarily when glycemic control is not achieved with oral antidiabetic agents alone or with monotherapy injectable agents.[1][3][5][6]
