Overview

A combination therapy of three antidiabetic agents used in the management of type 2 diabetes mellitus. Insulin glargine is a long-acting recombinant human insulin analog that provides basal glycemic control by stimulating peripheral glucose uptake and inhibiting hepatic glucose production. Exenatide is a glucagon-like peptide-1 (GLP-1) receptor agonist that enhances glucose-dependent insulin secretion, suppresses inappropriate glucagon secretion, slows gastric emptying, and promotes satiety. Metformin is a biguanide that decreases hepatic glucose production and improves peripheral insulin sensitivity. This triple regimen targets multiple pathophysiological defects in type 2 diabetes to improve glycemic control when oral agents alone are insufficient[1][4][5]. The combination has been studied in phase 3 clinical trials for patients with inadequate glycemic control on prior therapies[1][5].
