It has a faster onset and shorter duration of action than regular human insulin due to a single amino acid substitution (proline to aspartic acid) that reduces hexamer formation, allowing for quicker absorption. This basal-bolus combination mimics physiological insulin secretion. In clinical research, such as the NCT02144441 trial sponsored by the University of Pennsylvania, this regimen has been evaluated for feasibility in clinician-supported patient self-management of inpatient insulin therapy.
