Tianmailin M (insulin lisargine) is a long-acting recombinant insulin analog developed by Hefei Tianmai Biotechnology Development for the treatment of diabetes mellitus. It is a novel molecule, designated as HT1002, that incorporates structural modifications characteristic of both insulin lispro (B28 Lys, B29 Pro) and insulin glargine (A21 Gly, B31 Arg, B32 Arg). This hybrid structure is designed to provide a stable, long-lasting basal insulin profile with a reduced risk of hypoglycemia compared to traditional basal insulins. By binding to the insulin receptor, it facilitates glucose disposal in skeletal muscle and fat while suppressing glucose production in the liver. As of 2021, the drug was in Phase 1 clinical development in China, with studies focusing on its pharmacokinetics, safety, and tolerability in healthy volunteers and patients with type 2 diabetes.
