insulin lispro + insulin glargine + metformin is a combination drug regimen for the treatment of type 2 diabetes mellitus. This regimen combines three agents: insulin lispro (a rapid-acting insulin analog), insulin glargine (a long-acting basal insulin analog), and metformin (a biguanide oral hypoglycemic). Insulin lispro provides bolus insulin coverage for postprandial glucose control, while insulin glargine supplies basal insulin to maintain fasting and between-meal blood glucose. Metformin acts primarily to lower hepatic glucose production, increase insulin sensitivity, and reduce glucose absorption from the gut. Together, the combination is intended to achieve tighter glycemic control in patients not adequately managed with oral agents or basal insulin alone[2][3].
