Overview

Insulin lispro and insulin aspart are both rapid-acting recombinant human insulin analogs used primarily for the management of diabetes mellitus. They are designed to mimic the physiological release of endogenous prandial (mealtime) insulin. Insulin lispro differs from regular human insulin by switching the positions of proline and lysine at B28 and B29 on the B chain; this modification accelerates absorption after subcutaneous injection. Insulin aspart substitutes proline with aspartic acid at position B28, also resulting in faster absorption compared to regular human insulin[3]. Both analogs reduce self-association into dimers/hexamers, leading to a quicker onset and shorter duration of action than regular human insulins[1][3]. Their primary indication is for glycemic control in type 1 diabetes mellitus and in some cases type 2 diabetes mellitus or gestational diabetes when oral agents are insufficient[7].
