Mixtard is a premixed human insulin formulation used for the treatment of diabetes mellitus. It contains two forms of recombinant human insulin produced in Saccharomyces cerevisiae by recombinant DNA technology. The mixture includes a fast-acting (soluble) component and an intermediate-acting (isophane/NPH) component in varying ratios (e.g., Mixtard 30 contains 30% soluble and 70% isophane). After subcutaneous injection, the soluble fraction acts within about 30 minutes to lower blood glucose rapidly after meals, while the isophane fraction provides prolonged basal control throughout the day. Insulin exerts its effect by binding to the insulin receptor on muscle and fat cells to facilitate glucose uptake and inhibits hepatic glucose output[1][2][5]. Indications include type 1 diabetes mellitus, type 2 diabetes when oral agents are insufficient or contraindicated, gestational diabetes, perioperative glycemic management, diabetic emergencies such as pre-coma/coma states or severe complications[3].
