Insulin peglispro is a novel long-acting basal insulin analog developed for the treatment of type 1 and type 2 diabetes. It consists of the rapid-acting insulin analog lispro covalently linked to a large polyethylene glycol (PEG) polymer, which increases its hydrodynamic size and delays absorption and clearance. This PEGylation results in a flat, prolonged activity profile with hepato-preferential action—meaning it more effectively suppresses hepatic glucose production while having reduced activity in peripheral tissues such as muscle and fat. Clinical trials showed that insulin peglispro provided better glycemic control with less nocturnal hypoglycemia compared to conventional insulins like glargine but was associated with increased liver fat content, elevated triglycerides, higher alanine aminotransferase (ALT) levels, and more frequent injection site reactions. Due to these safety concerns related to lipid metabolism and liver effects, development was discontinued[1][3][4][6][7].
