Insulin regular human U-500 is a highly concentrated formulation of recombinant human insulin containing 500 units per milliliter—five times the concentration of standard U-100 (100 units/mL) insulins. It is indicated as an adjunct to diet and exercise to improve glycemic control in adults and children with type 1 or type 2 diabetes mellitus who require more than 200 units of insulin daily due to severe insulin resistance. The mechanism of action involves substituting for deficient endogenous insulin, thereby facilitating glucose uptake into cells and reducing blood glucose levels. Its pharmacodynamic profile reflects both prandial (mealtime) and basal (background) activity; it begins working within 30 minutes after injection, peaks similarly to standard regular insulins, but has a prolonged duration of action up to 24 hours due to its high concentration.
