Overview

This investigational combination cell therapy, developed by the Institute of Kidney Diseases and Research Centre (IKDRC) and the Dr. H.L. Trivedi Institute of Transplantation Sciences, consists of insulin-secreting cells differentiated in vitro from adipose-derived mesenchymal stem cells (IS-AD-MSCs) and donor-derived hematopoietic stem cells (HSCs). The therapy is primarily investigated for patients with Type 1 Diabetes Mellitus (T1DM), particularly those with end-stage renal disease (ESRD) requiring kidney transplantation. The IS-AD-MSCs are differentiated to express pancreatic markers such as Pax-6, Isl-1, and pdx-1 to replace lost beta-cell function and regulate blood glucose. The co-infusion of HSCs is designed to induce mixed chimerism, re-educating the recipient's immune system to recognize the transplanted cells and kidney as 'self,' thereby preventing autoimmune destruction and transplant rejection while minimizing or eliminating the need for long-term immunosuppression. The treatment is typically administered via a multi-route protocol involving the portal circulation, thymic circulation, and subcutaneous tissue.
