Insulin semilente is an intermediate-acting form of insulin used to manage diabetes mellitus. It is an amorphous zinc-insulin suspension that differs from other intermediate insulins like NPH in its absorption kinetics and duration of action. The addition of zinc causes the formation of larger crystals, which dissolve more slowly after subcutaneous injection, resulting in a delayed onset and prolonged effect compared to regular (short-acting) insulins. Insulin's mechanism involves binding to the insulin receptor on target tissues (primarily liver, muscle, and adipose tissue), promoting glucose uptake and storage while suppressing endogenous glucose production. Semilente was historically used for glycemic control in both type 1 and type 2 diabetes but has largely been discontinued or replaced by newer formulations due to variability in absorption and advances in recombinant human insulins[1][4][6].
