INT-787 is a hydrophilic, semisynthetic bile acid–derived small molecule that acts as a selective next-generation agonist of the farnesoid X receptor (FXR). It is being developed by Intercept Pharmaceuticals for the treatment of severe alcohol-associated hepatitis (sAH), a life-threatening liver condition with limited treatment options. INT-787 has demonstrated greater efficacy than obeticholic acid (OCA) in preclinical models, showing significant reductions in liver inflammation, fibrosis markers, and improvement in gut barrier function. The drug is currently undergoing Phase 2a clinical trials to assess its safety, tolerability, pharmacokinetics, and efficacy in patients with sAH.
