Intensified antiretroviral therapy (David Garcia Cinca) refers to a specific, multi-drug treatment protocol evaluated in the VIRECURE clinical trial for the treatment of acute HIV-1 infection. Developed under the leadership of David Garcia Cinca, this regimen is characterized by the extremely early administration of a potent combination of antiretroviral agents—including nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs), an integrase strand transfer inhibitor (INSTI), a protease inhibitor (PI) with a pharmacokinetic booster, and a CCR5 antagonist. The protocol is divided into three phases: an initial 48-hour induction with tenofovir, emtricitabine, dolutegravir, darunavir, ritonavir, and maraviroc; a three-month continuation with abacavir, lamivudine, dolutegravir, darunavir, ritonavir, and maraviroc; and a final nine-month maintenance phase with abacavir, lamivudine, and dolutegravir. The primary objective of this intensified approach is to drastically reduce the establishment of the viral reservoir in patients identified within 20 days of infection (Fiebig stages I-II), with the ultimate goal of inducing a functional cure.
