Intensive LDL-C targeting (I-OLD trial)

The Intensive targeting group refers to a specific therapeutic strategy evaluated in the I-OLD (Intensive LDL-C targeting in elderly patients with cardiovascular disease) Phase 4 clinical trial sponsored by Yonsei University. This regimen aims to achieve a low-density lipoprotein cholesterol (LDL-C) target of less than 55 mg/dL in elderly patients (aged 75 years and older) with documented cardiovascular disease. The pharmacological approach involves the use of high-intensity statin therapy, primarily atorvastatin (ranging from 5 mg to 80 mg), with the potential addition of ezetimibe or PCSK-9 inhibitors if the target LDL-C level is not reached with the maximum tolerated dose of atorvastatin. This strategy is compared against a conventional therapy group receiving moderate-intensity statin therapy to determine the clinical outcomes of aggressive lipid lowering in the elderly population.
