This intensive medical management (IMT) protocol is a multi-modal therapeutic strategy developed by the Post Graduate Institute of Medical Education and Research (PGIMER), Chandigarh, specifically for the management of obese patients with type 2 diabetes mellitus. It was utilized as the medical comparator arm in clinical trials evaluating the efficacy of laparoscopic sleeve gastrectomy. The protocol consists of a combination of pharmacological agents—including the GLP-1 receptor agonist exenatide, the biguanide metformin, and the long-acting basal insulin analog insulin detemir—integrated with a very low-calorie diet (VLCD) providing approximately 800 kcal/day. The intervention aims to achieve aggressive glycemic targets (HbA1c < 7%) and significant weight loss by simultaneously targeting insulin secretion, hepatic glucose production, and caloric intake.
