Intepirdine (SB-742457) is a small molecule drug that acts as a highly selective antagonist of the serotonin 5-hydroxytryptamine 6 (5-HT6) receptor. It was developed for the treatment of cognitive impairment associated with Alzheimer's disease and other dementias. The mechanism of action involves blocking the 5-HT6 receptor in the brain, which is thought to enhance neurotransmitter release and improve cognition and memory. Intepirdine was originally developed by GlaxoSmithKline and later by Axovant Sciences (now Sio Gene Therapies). Despite initial promise in early clinical trials suggesting potential benefits on cognition when used alone or as an adjunct to donepezil in mild-to-moderate Alzheimer's disease, subsequent larger phase III studies failed to demonstrate significant efficacy over placebo. As a result, development for Alzheimer's disease and dementia has been discontinued[4][7][9].
