This is a combination therapy consisting of interferon alfa-2a, a recombinant cytokine with immunomodulatory and antiproliferative properties, and isotretinoin (13-cis-retinoic acid), a retinoid that modulates cell differentiation and proliferation. The combination has been investigated primarily for its antitumor effects in various cancers. Interferon alfa-2a acts by binding to type I interferon receptors on cell surfaces, activating intracellular signaling pathways that result in antiviral, antiproliferative, and immunomodulatory effects. Isotretinoin influences gene expression through retinoic acid receptors (RARs), promoting cellular differentiation and apoptosis while inhibiting proliferation. Preclinical studies suggest the two agents may act synergistically to inhibit tumor growth via enhanced S-phase arrest, increased apoptosis, upregulation of p53 and Fas/CD95 death receptor pathways[3]. This regimen has shown activity in squamous cell carcinomas of the skin and cervix[1], head and neck[2][3], lymphoid malignancies[7], as well as being studied in metastatic colorectal adenocarcinoma[1][4][6] and melanoma[5].
