Interferon gamma-1b is a recombinant form of the naturally occurring cytokine interferon gamma, which plays a critical role in the innate and adaptive immune response. It functions by binding to the interferon gamma receptor complex, composed of IFNGR1 and IFNGR2 subunits, thereby activating the Janus kinase (JAK)-signal transducer and activator of transcription (STAT) pathway. This activation leads to the transcription of genes that enhance antigen presentation, activate macrophages, and promote Th1 cell differentiation. In the context of a Phase II clinical trial (NCT00786643) sponsored by the Accelerated Community Oncology Research Network (ACORN), interferon gamma-1b was investigated for its ability to sensitize metastatic colorectal cancer cells to the cytotoxic effects of 5-fluorouracil (5-FU) and leucovorin. This synergy is hypothesized to occur through the upregulation of the Fas receptor and other components of the extrinsic apoptosis pathway. While interferon gamma-1b is FDA-approved for the treatment of chronic granulomatous disease and malignant osteopetrosis, its development for metastatic colorectal cancer in this specific combination was terminated due to slow patient accrual.
