**interferon gamma-1b + antimycobacterials** is a combination regimen utilizing recombinant interferon gamma-1b (a cytokine) and one or more antimycobacterial agents such as azithromycin, ciprofloxacin, ethambutol, or rifampin. Interferon gamma-1b acts as an immunomodulator: it is a glycoprotein produced primarily by activated T lymphocytes (Th1 subset), with potent activity in activating macrophages and enhancing their ability to kill intracellular pathogens such as Mycobacterium avium complex (MAC). It increases the expression of major histocompatibility complex (MHC) antigens, Fc receptors, and stimulating macrophage production of reactive oxygen species and nitric oxide, leading to improved host immune response against mycobacteria. Antimycobacterials act directly to inhibit bacterial growth via varying mechanisms targeting mycobacterial cell wall synthesis, protein synthesis, or nucleic acid function. This combination is studied primarily as adjunct therapy for atypical mycobacterial infections (especially in immune-compromised or genetically susceptible patients) and has shown improved clinical, bacteriological, and radiographic outcomes compared to antimycobacterials alone.
