Topical Interleukin-1 Receptor Antagonist (IL-1Ra) is an investigational biologic therapy developed by Dr. Reza Dana for the treatment of corneal neovascularization. IL-1Ra is a naturally occurring protein that functions as a competitive inhibitor of the interleukin-1 receptor, specifically blocking the pro-inflammatory activities of interleukin-1 alpha (IL-1α) and interleukin-1 beta (IL-1β). These cytokines are critical mediators of the inflammatory response that drives pathological blood vessel growth in the cornea. The drug was formulated as a topical solution (at concentrations of 2% or 5%) in a hyaluronic acid vehicle to enhance ocular surface retention. Clinical development included a Phase 1/2 crossover study (NCT00915590) to evaluate safety and efficacy in reducing corneal vessel growth, although the trial was terminated early due to recruitment challenges. Preclinical data suggested that early intervention with topical IL-1Ra could significantly suppress neovascularization and improve the outcomes of corneal transplantation.
