This is an experimental adoptive cell-based immunotherapy consisting of natural killer (NK) cells harvested from donor liver graft perfusate (from either deceased or living donors). The therapy is developed by Dr. Seigo Nishida at the University of Miami Miller School of Medicine in collaboration with Hiroshima University. During the manufacturing process, the donor-derived NK cells are stimulated in vitro with interleukin-2 (IL-2) to enhance their cytotoxic potential and treated with anti-CD3 antibodies to deplete T cells, thereby reducing the risk of graft-versus-host disease (GVHD). The therapy is specifically designed to prevent the recurrence of hepatocellular carcinoma (HCC) in patients following liver transplantation. The mechanism of action involves the upregulation of tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) on the NK cells and the secretion of interferon-gamma, which together facilitate the targeted apoptosis of residual HCC tumor cells.
