Interleukin-5 chimeric antigen receptor T (IL-5 CAR-T) cells are an investigational cell therapy designed to target and deplete eosinophils for the treatment of severe eosinophilic diseases. These T cells are genetically engineered to express a chimeric antigen receptor (CAR) that typically utilizes the interleukin-5 (IL-5) cytokine as its extracellular binding domain. This configuration allows the T cells to specifically recognize and bind to the interleukin-5 receptor alpha (IL-5Rα, also known as CD125), which is selectively and highly expressed on the surface of eosinophils and their precursors. Upon engagement with IL-5Rα, the CAR-T cells are activated to eliminate the target eosinophils through direct cytotoxic mechanisms. This approach is being evaluated in clinical trials for hypereosinophilic syndrome (HES) and is being researched for other conditions such as severe eosinophilic asthma and eosinophilic granulomatosis with polyangiitis (EGPA), potentially offering a more comprehensive and durable depletion of eosinophils than current monoclonal antibody therapies like mepolizumab or benralizumab.
