Overview

Intismeran autogene (mRNA-4157/V940) is an investigational, personalized mRNA-based individualized neoantigen therapy (INT) being co-developed by Moderna and Merck (MSD). It is designed to stimulate a patient-specific immune response by encoding up to 34 unique neoantigens identified through genomic sequencing of the patient's resected tumor. Using a proprietary algorithm, the therapy selects the most immunogenic neoantigens, which are then delivered via mRNA encapsulated in lipid nanoparticles. Once administered, the mRNA is translated endogenously, allowing the immune system to recognize and prime T cells against the specific mutations present in the patient's cancer. In the context of renal cell carcinoma, it is being evaluated as an adjuvant treatment in combination with the anti-PD-1 checkpoint inhibitor pembrolizumab to prevent disease recurrence following complete resection.
