The therapy integrates three distinct components: intra-arterial catheter infusion chemotherapy (delivering high concentrations of cytotoxic agents like oxaliplatin and 5-fluorouracil directly to the gastric tumor), systemic chemotherapy (to address potential micrometastases), and sodium bicarbonate. The inclusion of sodium bicarbonate is based on the Targeted Intra-arterial Lactic Acid-infused (TILA) concept, which hypothesizes that alkalinizing the acidic tumor microenvironment can overcome acid-induced drug resistance and enhance the cytotoxic efficacy of chemotherapeutic agents. This approach aims to improve local tumor control and overall survival in patients with advanced, non-surgical gastric cancer.
