Overview
This experimental combination regimen, developed by the Second Affiliated Hospital of Zhejiang University School of Medicine, is designed for the treatment of unresectable gastric cancer. The therapy integrates three distinct components: intra-arterial catheter infusion chemotherapy (delivering high concentrations of cytotoxic agents like oxaliplatin and 5-fluorouracil directly to the gastric tumor), systemic chemotherapy (to address potential micrometastases), and sodium bicarbonate. The inclusion of sodium bicarbonate is based on the Targeted Intra-arterial Lactic Acid-infused (TILA) concept, which hypothesizes that alkalinizing the acidic tumor microenvironment can overcome acid-induced drug resistance and enhance the cytotoxic efficacy of chemotherapeutic agents. This approach aims to improve local tumor control and overall survival in patients with advanced, non-surgical gastric cancer.
