Intralipid is a sterile, non-pyrogenic fat emulsion intended for intravenous administration as a source of calories and essential fatty acids. It is traditionally used in parenteral nutrition for patients who are unable to receive adequate oral or enteral intake. The formulation typically consists of soybean oil, egg phospholipids, and glycerin. Beyond its nutritional applications, Intralipid has gained significant attention in reproductive medicine for its immunomodulatory properties. It is hypothesized to suppress the activity and cytotoxicity of natural killer (NK) cells and modulate the Th1/Th2 cytokine balance, which may improve pregnancy outcomes in women with recurrent implantation failure (RIF) or recurrent pregnancy loss. Clinical trials, such as those conducted by King Fahd Medical City, have evaluated its empiric use in women undergoing intracytoplasmic sperm injection (ICSI) who have a history of unexplained RIF.
