Intranasal insulin aspart is an investigational biologic formulation being studied by the Albert Einstein College of Medicine for the prevention of post-operative delirium (POD) and post-operative cognitive decline (POCD) in elderly patients undergoing cardiac surgery. Insulin aspart is a rapid-acting insulin analog that, when delivered intranasally, bypasses the blood-brain barrier to directly access the central nervous system. The drug acts as an agonist at the insulin receptor, where it is believed to exert neuroprotective effects and improve cognitive function by modulating neuronal metabolism and synaptic plasticity. A Phase 2 clinical trial (NCT01561378) has evaluated its efficacy in reducing acute cognitive dysfunction in patients undergoing coronary artery bypass graft or valve surgery requiring cardiopulmonary bypass.
