Intranasal Modified Temozolomide refers to novel formulations of the alkylating agent temozolomide (TMZ) designed for direct nose-to-brain delivery using nanotechnology. These include derivatives such as temozolomide hexadecyl ester (TMZ16e) and conjugates like anti-ephrin type-A receptor 3 (EphA3)-modified nanoparticles or gold nanoparticles. The modifications aim to enhance brain targeting, bypass the blood–brain barrier, reduce systemic toxicity, and overcome drug resistance in glioblastoma multiforme (GBM). Intranasal administration of these nanoformulations has demonstrated improved cellular uptake in glioma cells, downregulation of O6-methylguanine-DNA methyltransferase (MGMT), reversal of TMZ resistance, increased apoptosis in tumor cells, and significantly prolonged survival in animal models compared to standard TMZ[1][2][4].
