Overview

Intravenous insulin + basal insulin refers to a therapeutic regimen investigated for the management of diabetic ketoacidosis (DKA). This protocol involves the simultaneous administration of a continuous intravenous insulin infusion (the standard of care for acute DKA) and subcutaneous basal insulin (such as insulin glargine). The rationale behind this combination, as studied by Tanta University, is to provide a steady baseline of insulin that prevents the recurrence of hyperglycemia and ketosis during the transition from intravenous to subcutaneous therapy, potentially reducing the length of stay in the intensive care unit (ICU) and lowering relapse rates. Insulin acts as an agonist at the insulin receptor, facilitating glucose uptake into cells and suppressing hepatic glucose production and lipolysis, thereby reversing the metabolic derangements of DKA.
