INVAC-1 + ibrutinib is an investigational combination regimen for the treatment of chronic lymphocytic leukemia (CLL), particularly in patients with minimal residual disease (MRD) following ibrutinib therapy[1][3]. INVAC-1 is a DNA plasmid-based therapeutic vaccine encoding a modified human telomerase reverse transcriptase (hTERT) protein. It is designed to stimulate the immune system to target cells expressing telomerase, a hallmark of many cancer cells, thereby aiding in the eradication of residual malignant cells. Ibrutinib is a small molecule Bruton tyrosine kinase (BTK) inhibitor approved for the treatment of several B-cell malignancies, including CLL. In this combination, INVAC-1 aims to enhance immunological clearance of CLL cells, while ibrutinib suppresses B-cell receptor signaling essential for leukemic cell proliferation and survival. The combination is being evaluated in phase II clinical trials for its efficacy in achieving MRD negativity and improving long-term outcomes in CLL[1][3].
