Invega Xeplion

Invega Xeplion is a long-acting injectable (LAI) atypical antipsychotic developed by Janssen Pharmaceuticals for the maintenance treatment of schizophrenia and schizoaffective disorder in adults. It consists of paliperidone palmitate, a palmitate ester prodrug of paliperidone (9-hydroxyrisperidone), which is the primary active metabolite of risperidone. The drug's therapeutic activity is primarily attributed to its potent antagonism of central dopamine D2 and serotonin 5-HT2A receptors. It also acts as an antagonist at alpha-1 and alpha-2 adrenergic receptors and H1 histaminergic receptors, which may contribute to its side effect profile. Formulated as a prolonged-release aqueous suspension for intramuscular injection, it is typically administered once monthly, providing a consistent release of the active moiety to improve patient compliance and reduce the risk of relapse compared to oral antipsychotics.
