Overview

IO-108 is a fully human IgG4 monoclonal antibody that targets leukocyte immunoglobulin-like receptor B2 (LILRB2, also known as ILT4), an inhibitory immune checkpoint receptor expressed on myeloid cells such as monocytes, macrophages, dendritic cells, and neutrophils. By blocking the interaction of LILRB2 with its ligands—including HLA-G, ANGPTLs, SEMA4A, and CD1d—IO-108 reduces cancer-associated immune suppression in the tumor microenvironment. This blockade promotes a proinflammatory phenotype in myeloid cells and enhances T-cell activation. IO-108 has demonstrated favorable safety and preliminary efficacy both as monotherapy and in combination with anti-PD-1 agents (such as pembrolizumab) across multiple solid tumor types. It is being developed primarily for advanced liver cancer (hepatocellular carcinoma) and other solid tumors by Immune-Onc Therapeutics in collaboration with Roche[1][3][5][8].
