IO-202 is a first-in-class, humanized IgG1 monoclonal antibody that targets leukocyte immunoglobulin-like receptor B4 (LILRB4, also known as ILT3), which is predominantly expressed on monocytes and monocytic blasts. Developed by Immune-Onc Therapeutics, IO-202 acts as an antagonist of LILRB4 and is designed to activate T cell cytotoxicity against cancer cells. Its mechanisms include activation of T cell cytotoxicity, induction of antibody-dependent cellular cytotoxicity (ADCC) and antibody-dependent cellular phagocytosis (ADCP), and reduction of leukemia tissue infiltration. IO-202 has received FDA Orphan Drug Designation for the treatment of acute myeloid leukemia (AML) and chronic myelomonocytic leukemia (CMML). It is currently in Phase 1 clinical trials for AML, CMML, and solid tumors[1][2][3][5][6][8].
