This is a **combination therapy** consisting of IO102 (an investigational immunomodulatory peptide), pembrolizumab (a monoclonal antibody targeting PD-1), carboplatin (a platinum-based chemotherapy agent), and pemetrexed (an antifolate chemotherapy agent). IO102 is designed to inhibit immunosuppressive mechanisms in the tumor microenvironment (including IDO-derived immunosuppression). Pembrolizumab is a checkpoint inhibitor that blocks PD-1, leading to increased T-cell mediated antitumor activity. Carboplatin induces DNA crosslinking and damage in tumor cells, resulting in cytotoxic effects. Pemetrexed inhibits multiple folate-dependent enzymes involved in the de novo synthesis of thymidine and purine nucleotides, disrupting DNA synthesis and cell proliferation. This regimen is primarily studied and used for advanced/metastatic non-small cell lung cancer (NSCLC), particularly nonsquamous NSCLC, and the addition of IO102 to standard-of-care pembrolizumab plus carboplatin and pemetrexed is aimed at enhancing immunotherapeutic response.
