**Iobenguane** is a synthetic guanethidine derivative and radiopharmaceutical agent used primarily for the diagnosis and treatment of certain neuroendocrine tumors, including pheochromocytoma and paraganglioma. It mimics norepinephrine in the body, accumulating in adrenergic tissue such as adrenal medullary cells. When labeled with radioactive iodine (either iodine‑123 or iodine‑131), it can be used for imaging (diagnosis) or targeted radiotherapy. The radioactive component delivers cytotoxic radiation to tumor cells that take up the compound, leading to cell death. The drug is approved for use in patients with advanced or metastatic pheochromocytoma or paraganglioma who are not candidates for surgery and require systemic therapy[1][2][3][4][7]. It is also being studied in other cancers.
