Iobenguane I-123 is a radiopharmaceutical diagnostic agent used primarily for imaging of neuroendocrine tumors and disorders of the adrenal medulla. It is a radioactive analog of norepinephrine (noradrenaline) that is selectively taken up by adrenergic nerve terminals via the noradrenaline uptake transporter. Once inside these cells, it accumulates in chromaffin granules and can be visualized using gamma-scintigraphic imaging due to its radioactive iodine isotope (I-123). The main clinical indications include detection and localization of primary or metastatic pheochromocytoma and neuroblastoma, as well as assessment of myocardial sympathetic innervation in certain heart failure patients. It may also be used for evaluation of paragangliomas, carcinoid tumors, adrenal medullary hyperplasia (including MEN syndromes), and medullary thyroid carcinoma. **Developer:** GE Healthcare
