iobenguane I-131 + vorinostat is an investigational combination therapy for recurrent or progressive high-risk neuroblastoma. It combines *iobenguane I-131* (also known as 131I-MIBG), a radiopharmaceutical that delivers targeted radiation to neuroblastoma cells, with *vorinostat*, a small molecule inhibitor of histone deacetylases (HDAC). Iobenguane I-131 specifically accumulates in neuroendocrine tumor cells due to its structural similarity to norepinephrine, leading to selective cell death from radioactive decay. Vorinostat sensitizes cancer cells to radiation by altering chromatin structure and making DNA more susceptible to damage. This synergy aims to improve outcomes in neuroblastoma patients resistant or unresponsive to prior therapies[1][2][3][4][5][6].
