IOC is a reduced-intensity conditioning (RIC) regimen developed by the National Cancer Institute (NCI) for patients with relapsed or refractory peripheral T-cell lymphoma (PTCL) undergoing allogeneic hematopoietic cell transplantation (allo HCT). The regimen is designed to facilitate donor cell engraftment while minimizing transplant-related mortality through a combination of targeted immunosuppression and low-dose chemotherapy. It consists of equine anti-thymocyte globulin (e-ATG), which depletes host T-cells to prevent graft rejection and graft-versus-host disease (GVHD); pentostatin, a purine analog that potently inhibits adenosine deaminase (ADA) to further induce lymphopenia; and low-dose oral cyclophosphamide, an alkylating agent that provides additional lymphodepletion and anti-lymphoma activity. This regimen is typically evaluated alongside other RIC protocols to optimize outcomes in T-cell malignancies.
