**Iodine-123 α-methyl-l-tyrosine** is a synthetic radiolabeled amino acid analog used as a radiopharmaceutical diagnostic agent, primarily for brain tumor imaging. It is transported predominantly by the System L amino acid transporter and accumulates in tumor tissues due to increased amino acid transport, but it is not incorporated into proteins. It is used in single-photon emission computed tomography (SPECT) to detect and differentiate recurrent gliomas from benign post-therapeutic lesions. Its uptake correlates with the proliferative activity of tumor cells, and it may aid in therapy planning for glioma patients. The mechanism is based on high tumor-specific uptake by the L-type amino acid transport system, associated with 4F2 antigen expression, but does not target a signaling or metabolic pathway directly[1][2][4].
