Iodine-124 iododeoxyuridine (124I-IUdR) is a radiolabeled thymidine analog used primarily as a positron emission tomography (PET) imaging tracer to visualize and quantify cellular proliferation in tumors. As an analog of thymidine, it is incorporated into the DNA of cells during the S-phase of the cell cycle. This property allows for the assessment of tumor growth kinetics and the distribution of the drug within the tumor microenvironment, particularly in malignant gliomas. It has been investigated for use via convection-enhanced delivery (CED) to bypass the blood-brain barrier. While primarily diagnostic, its incorporation into DNA also suggests potential as a targeted internal radiotherapeutic or radiosensitizer. Research is largely conducted in academic settings, such as Odense University Hospital and the University of Southern Denmark.
