Iodine-131 anti-endoglin monoclonal antibody A8 is an investigational radiopharmaceutical developed for the treatment of hepatocellular carcinoma (HCC). It consists of the monoclonal antibody A8, which specifically targets endoglin (ENG, also known as CD105), conjugated to the radioisotope Iodine-131. Endoglin is a cell membrane glycoprotein that is highly expressed on the angiogenic endothelial cells of solid tumors and some cancer cell lines, making it a viable target for vascular-homing therapies. The mechanism of action involves the antibody binding to endoglin-expressing cells within the tumor microenvironment, thereby delivering localized beta-radiation from the Iodine-131 isotope. This radiation induces DNA damage and subsequent cell death in both the tumor vasculature and the tumor cells themselves, leading to suppressed tumor growth and reduced tumor burden. Preclinical research conducted by institutions in Taiwan has demonstrated its efficacy in animal models of HCC.
