Iodine-131-chTNT is a radiolabeled monoclonal antibody therapy targeting DNA/histones in cancer cells, delivering targeted beta radiation (from iodine-131 isotope) for cell killing[1][5]. Its core antibody is chTNT (chimeric Tumor Necrosis Treatment), referred to by USAN as 131I-derlotuximab biotin[1]. The mechanism exploits selective binding to denatured nuclear antigens present in tumor tissue, allowing irradiation of cancer cells with minimal impact on healthy tissue[1][3][5]. Originally developed for brain cancers such as glioblastoma multiforme (GBM), it is directly infused into tumors via stereotactic catheters[3], and has received orphan and fast-track status from the FDA for GBM[1]. Development for GBM was discontinued, but ongoing trials and clinical use continue in China for lung, brain, and liver cancers under the brand name Vivatuxin[1]. Developed to treat aggressive cancers with advanced local delivery methods, it is representative of targeted radiopharmaceutical therapy in oncology.
